In Sonoma County, we are trying to become the healthiest county in California by 2020. Our shared vision through Upstream is to eliminate poverty and ensure equal opportunity for quality education and good health in nurturing home and community environments. We work collectively to achieve this vision through a wide array of public and private health, human service, education, and criminal justice policies and interventions.
In Sonoma County, many of our government and community-based agencies collect information about the clients that we serve. This information helps us to understand the needs of our community and how our services promote safety, self-sufficiency, and well-being for the children, families, and individuals in our community. Unfortunately, this information is inherently incomplete. No single organization can address all the challenges that we are collectively trying to address in our county. And more often than not, the clients one agency serves, are also being served elsewhere. Subsequently, information about the needs and successes of our children, families and individuals is scattered and unavailable to help ensure a collective understanding of our strengths and weaknesses as a community.
We have a vision for shared measurement. We believe we can more fully understand the successes we are all achieving together. We believe this infrastructure will help us to eliminate disparities and achieve social justice.
